Adam Boehler, CMS deputy administrator and director of CMMI, told an audience at last week\u2019s HIMSS19 conference in Orlando that the agency wants to \u201cblow up\u201d fee-for-service.

\u201cWe\u2019re going to do everything we can to change that system,\u201d said Boehler, who joined CMS last April. \u201cWe can\u2019t support a system that\u2019s based on volume\u2014because you get what you pay for. That is a goal underlying everything.\u201d

CMMI, which was created by the Affordable Care Act to test innovative payment and service delivery models leveraging health IT, is looking first at transforming primary care, which Boehler describes as the \u201cbackbone\u201d of America\u2019s healthcare system.

\u201cThe nice thing about primary care is it\u2019s a small amount of spend, but it controls so much more,\u201d observed Boehler. \u201cAnd, so, it\u2019s something that you can easily invest in and that can respond very well to outcomes.\u201d

He noted that many physician groups in Medicare Advantage are \u201ctaking full accountability,\u201d and CMMI is looking to give physician groups \u201cthat are ready to take full accountability\u201d in traditional Medicare fee-for-service the opportunity to also improve quality and drive down healthcare costs.
